Service Provider Agreement

ONYX MOBILE BEAUTY SALON — SERVICE PROVIDER AGREEMENT (v1)

1. Independent contractor status. You are an independent contractor, not an employee of Onyx Mobile Beauty Salon. Nothing in this agreement creates an employment, partnership, agency, or joint-venture relationship.

2. Customer satisfaction. You are solely responsible for the quality of the service or product you deliver and for customer satisfaction. You will resolve customer concerns directly and in good faith.

3. Limitation of liability. Onyx Mobile Beauty Salon is not liable for any damages, injuries, losses, or claims arising from your services, products, conduct, or presence at a customer's location. You agree to indemnify and hold Onyx harmless from any such claims.

4. Required documents. Service providers must upload valid (a) proof of liability insurance, (b) government-issued photo ID, and (c) a current service license before being approved. Providers of beauty supplies only are not required to upload liability insurance or a service license, but must still upload a photo ID.

5. Platform fee. You will pay Onyx a platform fee equal to 30% of the price of each service performed or product sold through the platform. Onyx will deduct this fee before remitting your payout.

6. Payment schedule. Payouts run weekly. The work week is Sunday through Saturday, and payouts for the prior week are issued on the following Wednesday.

7. Cancellation split. If an appointment is cancelled within 3 hours of the scheduled start time or the customer no-shows, the customer is charged 50% of the service price and the 30% platform fee applies to that amount.

8. Live tracking. When you are en route to a booking, you agree to share your live location with Onyx and the customer for safety and coordination.

9. Standards & conduct. You agree to arrive on time, use professional-grade tools and products, and comply with all local health, safety, and licensing regulations. Harassment, discrimination, unsafe behavior, or bringing unapproved third parties to a service address may result in immediate removal from the platform.

10. Taxes. You are responsible for reporting and paying all applicable taxes on income earned through Onyx.
